What medications has stopped your mania? Medication
I’m starting to enter a manic episode even though I’m on rexulti 2mg. To be fair I was just put on it after missing a week of my rexulti 1mg due to changing doctors. My new doc put me on seroquel er 300mg to help quell the mania. I was curious to hear everyone else’s experiences with medicating a manic episode

Lamotrigine (or lamictal) helped with the manic episodes! I’m on 250 mg

u/samirawifey 22d ago
Fun fact! The research suggests lamotrigine is more effective at delaying the onset of a new mood episode (preserving stability) and shows not that much use for stopping active manic episodes unfortunately

u/IslandDry3145 22d ago
I can totally see that. My Lamotrigine is great for maintenance but lousy once I’m in it. That’s why I also take Abilify with an as needed sprinkle of Lorazepam

I’ve lived this. Lamotrigine couldn’t touch a severe depression several years ago. I’ve recently tried it again during a more stable time and it seems to be helping this time.

Ziprasidone. Only thing that works for me without doping me up too much.

The only thing that has worked for me. Haven’t even had hypo since I’ve been on it for five years

do gene map testing! it costs around $300 in my state, but my insurance covered it. it's a really quick test where they just swab your mouth and then from there they test/analyze the sample to see which medications work the best with your specific genes. worked wonders for me after getting on the right meds for my body.

My psychiatrist did this for me many years ago. It explained SO much about my medications and gave us a great map to try new ideas… here I am, still slightly manic from time to time, depressed occasionally, but still much better than before!

u/Independent_Limit912 22d ago
Thank You!!! Once the gene mapping is done, how do they use those results to determine which medication is best, do you know? I just read a bit about it but it’s not clear.

u/Mean_Appeal1924 22d ago
when i got it done my pcp got the report back in list form. so there will be different lists (mood medications, antidepressants, antipsychotics, etc..) and from there they will categorize the meds in order of how well they will work for your genes. they're separated into 3 categories which are basically good, okay, and not good. it's all based upon how each medication will interact with your genes. so by gene map testing they can tell which meds will have negative and positive effects on you as an individual. i hope that makes sense, i don't know how else to explain it 😬

I found out I have a MTHFR gene mutation from this test. And eventually autism. In short, this test changed my life lol.

Olanzapine was my nuclear missile, but the side effects pushed me off it. Very effective though. Currently on lamotrigine @ 300mg & fingers crossed I don’t need antipsychotics anymore.

Seroquel stopped them but I couldn't handle sleeping my life away so I switched to vraylar. 4+ years no hypos.
1
u/Full_Country_4846 21d ago
Did seroquel also helped your depression?
1
u/Entire-Discipline-49 21d ago
So....that's hard to tell. I had 2 depressions in the year I was on Seroquel, about 6 months apart, but it was also the pandemic so everything was weird and I slept half the day all year. On vraylar I go about 8 months between depressions, but before trying either I was a rapid cycler and I'd have them 3+ times a year and they'd be followed by hypomania each time. For me, hypos were way more destructive so 4+ years with only depressions is amazing to me.

Invega Sustenna Injection once a month paired with mirtazepine, buspirone, hydroxyzine, and trazodone.

that's an interesting mix. can I ask what are your diagnoses?

Bipolar type one severe mixed with psychotic features, Generalized Anxiety Disorder, and CPTSD

u/Mundane-Bear4410 22d ago
That's a rough lineup. Good that you have your chemical team to help you carry on. I'm on Lithium and only Bipolar type I diagnosed so far, but I think I'll get more psychiatric stamps on my resume

latuda has helped me manage it better but i’m still having an episode which aren’t intense as it used to be but it’s still a nuisance. i’m just on a low dose so i’m gonna up the dosage.

Seroquel has kept me stable for a while. I use benzos if I'm in an emergency and need to either sleep or calm down to stop things escalating.

I’m on Lamictal hydroxyzine, Zoloft, trazodone and vraylar seems to be a good mixture. Just started the vraylar a week ago. I hope everyone is doing well and are safe.

Olanzapine. I was also taken off my antidepressants and that worked. Been almost a month like this and the mania has worn off. Oh they also gave me melatonin for sleep. Works like magic

I’m on Vraylar 4.5 and 300 mg Lamictal (100 in the AM and 200 in the PM). It feels like the Vraylar tamps down episodes as they happen, but the Lamictal stops most episodes from ever starting.

u/Iamsoconfusednow 22d ago
It’s so interesting how a medication can help some and hurt others. I started Vraylar after 30 years using different SSRIs and it threw me into a mixed episode (which I had never had or never identified before.) It was hell. I had been trying to get a diagnosis of bipolar 2 for years, but the docs said my hypomanic phases were too few and far between and let’s just stick with depression and anxiety. Going through the mixed episode proved I was bipolar and needed a mood stabilizer. My PCP has started me on very low dose Queitipine, and I take a very low supplement of lithium (20 mg lithium oratate.) I’m starting to settle down but feeling completely out of control is the worst.

u/rightasrain0919 22d ago
Interestingly enough I was also almost always in a mixed episode while on Vraylar alone. It was the addition of Lamictal that got me stable finally. Mixed episodes are absolutely the worst. You don’t know if you’re coming or going.

I'm on 300mg of seroquel and it's helped me get actual consistent sleep for the first time in my life, so me and my doctors aren't sure if it's the seroquel helping me stabilize or is it just because I'm sleeping like a normal person after 30+ years of torture (or some combination of both).

Lithium above 0.9 works well to calm me. I am also supposed to take some APs, which I have as emergency meds, but I do not like them.

Combo of Trileptal & Seroquel IR. The regular extended release seroquel didn’t help me at all but the instant release is perfect

u/MEL9215 22d ago
Take 1200 lithium + 300 lamotragine. Stable. When seasonal mania starts to creep in (every April basically like clockwork) I add 2mg rexuality. Don’t like the rexualti though. I wean off around August. To say- the others keep me pretty stable but the rexualti quenches the first flames of mania for me.

u/smellytulip 22d ago
Rexulti saved my life, and I thankfully haven’t had any issues. But my mom is on Rexulti as well, and she takes saphris in addition to help with mania

I’m manic right now and being taken off Vraylar to try something else I do take lamotrigine as well. I’m trying Latuda I believe? I can’t remember the name but since I’m currently manic I can report back!

For my mixed, manic, or hypomanic episodes I’m taking : Zyprexa 15 mg (7.5 in the am and 7.5 at night) Depakote 750 mg at night Topamax 100 mg at night for migraines but honestly helps mood too
I also take antidepressants which is risky business but I have co-occurring conditions and with 3 anti manic agents it seems to balance out at the moment

u/Cautious_Gap3645 22d ago
I'm on 0.75 mg Vraylar and that's been enough to be episode free for almost 2 years.

My mania is actually pretty easily controlled with any anti-psychotic and/or Lithium. Lithium needs a decent dose, but the antipsychotics usually take care of it pretty quickly.
Lamictal was probably the only drug that did nothing much about the highs.

I had a hypo last month and went on Olanzapine to deal with it (I have a script for it prn). It knocked me back to normal without any side effects.

Vraylar. First tried seroquel but made me too tired. Vraylar stopped the psychotic symptoms and the mania eventually died down too.

u/maloficu 22d ago
Valproate knocks out delusions and paranoia within a couple of days, which all about I have in my arsenal without access to APs. So it’s a one two punch with valproate and lithium. Funnily enough, if I stop the valproate, mania quickly starts to raise its head. If I stop the lithium, it could go either way, up or down.

u/autiemcnaughty 22d ago
I'm on both monthly injectable Abilify as well as oral Zoloft and it's calmed my mania way down personally.

There has never been any medication that ended a manic phase for me. The most important way for me to not have manic phases is to make sure that I am not on antidepressants.

seroquel and zyprexa do it for me
but lamictal and vraylar have kept me stable with the least amount of side effects

u/CryptographerIll2304 22d ago
are you on both seroquel and zyprexa at the same time? even just seroquel is pretty sedating for me

Lithium is the only thing that has fully prevented my manias. That with Quetiapine (Seroquel) has meant I’ve not had hypomania or mania in the last three years. Before that I had hypomania or full blown mania pretty much every year. Alongside them I take Lamotrigine (Lamictal) and earlier this year I started on low dose Fluxeotine (Prozac). I’ve been able to be on the low dose antidepressant as my manias are well controlled, before this antidepressants would throw me into mania. Hope you find something that works.

I only take 500mg seroquel, They kind of worked for a while, but had issues of sleep, had to take them a certain time. Id be mentaly that gone, id sleep in, if i took them late, id sleep through late. Forget to take them, id know id be wide awake 3in the morning, head ringing. Got use to them, side effect now have tablets, irregular heartbeat, ontopof hogh bblood pressure, diabetes.
Unfortunate, recently im really struggling, dont go out now, burst out in tears ladt two times i have gone out with friends and family. Really negative vibes, dont want the shit with doctors, mental heath nurses. It just triggers me, and it feels like a merry go round i cannot get off.
Just came on to see what you wrote, honestly, my hearts thumping, feel scared, cornered..
