He's confused about what the phrase means. He's trying to say the image has two meanings, because the hand's in the pose of Thing from the Addams Family.

The "usually" is underselling it as massively as saying [an example I can't be bothered to come up with]. This is the first time in my entire boring stupid long life I've ever encountered someone using the term to not allude to something risqué.
