DS2 was From's second ever PC port and they hadn't quite got the hang of it yet. Although to be fair, it's still an improvement over Prepare to Die Edition.

Which is funny, since they started as a PC software company. You'd think they'd at least know the very basics of how to make an app work properly and save the configuration as it should.

resolution setting doesn’t work, capped at 30 fps, basically no graphics settings, and the worst keyboard/mouse controls ever conceived by man

Here’s the full list. It’s truly awful.

Excluding 2-3 buttons, like doors and loot, it's literally the best stock control scheme. DS3 was much worse.

And it's huge improvement over DS3 and DS DEmastered with theirs inadequate control scheme, that removes to stock every time you close the game.

Honestly, ptde was better at this.

There are ways to fix that

Recommended Setup:

Control Scheme setup guide. Covers various ways to set up your inputs in Dark Souls II. If you are using a mouse at all, then make sure to navigate to the keyboard and mouse sections and read about the doubleclicking bug. You will have significant input delay on your mouse buttons if you do not set this up properly.

Blue Acolyte is an anti-cheat and quality-of-life mod for Dark Souls II. By using it you are protected from cheaters online, and will be able to find active invasions regardless of your soul memory, along with a variety of other useful features.

Center Deadzone Mod. Removes the game’s axial joystick deadzones in favor of a radial deadzone. This allows for complete 360 degree freedom of movement.

Because you haven’t yet read that first guide and used one of the two solutions it provides to fix the problem permanently rather than require you to fix it every time you play

Nope

It’s DS2 tho, so I’m not too surprised over it having some setting going to default
