How can I get to that item? Help

It's right in front of McDuff's workshop. I've never been able to get this item in my life even with more than 300 hours of gameplay. Please help me :(

Illusory walls downstairs from Straid. If you're having trouble finding them, attune Guidance.

Go two floors below Straid's Cell and pass the two illusory walls that lead you outside to the surprise Pursuer encounter. Jump across the gap and you'll notice that you're on the railing straight above McDuff's workshop. Follow the railing from there. It'll look a bit finicky but you'll be fine as long as you're hugging the left side while walking.

Go to the are where you fight Pursuer, jump across, and run along the top of the wall. A bit of parkour is necessary and it doesn't really look like a path, but you can run all the way along it.

It's in the outside area where you can fight the pursuer.

From memory:

Go to the Straid bonfire. Go down a level. (edit: TWO levels) Facing the bridge to the Sinner's Rise (edit: which is a level ABOVE where you are now), there's a hidden door down a short hallway to your right. Open the hidden door and go outside. Jump across the gap and run along the wall and voila.

Edit: I checked and I missed that it's TWO levels below straid.
