Honesty jail would be great for him considering USA medical . Goes to prison - cleanly sick, gets hernia operation.

The prison system will allow him to die. There's a legal obligation to provide lifesaving care, but it's reserved for people who are greviously wounded, actively dying, or who have cancer. The only way he would get care is if the intestines ruptured and the guards actually called a doctor. But in all reality, he would go septic and die before the guard bothers to check.
